In addition, we function as the Child and Family Connections agency for DuPage, Kane and Kendall Counties.
This service works with families with children who are eligible for services through the Illinois early Intervention system.

org/ We are seeking a full-time Service Coordinator to join our Life Management Services team working in our Lisle, IL location.
This position generally works Monday-Friday between 8 am - 5 pm.
The ideal candidate will provide individuals with disabilities with case management, trustee and/or guardianship services.
Services may include review and oversight of needs and treatment, obtain goods and services that will benefit the individual, and advocate for and make decisions on behalf of guardianship clients when necessary.
The SC will follow ethical guidelines and possess excellent interpersonal skills and good judgement.
Duties/
Responsibilities:
Provide service coordination and advocacy to a caseload of clients.
Visit individuals as specified in Service Agreement or in our capacity as legal guardian.
Establish and maintain contact with family/friends and relevant professionals working with our clients.
Assist clients with personal shopping, searching for and securing vendors, or linkage.
Provide informed consent as needed for individuals under guardianship.
Arrange services and if necessary, accompany or escort client.
Monitor treatment and care plans, attend individual staffing meetings.
Be available for on-call emergency coverage.
Attend trainings and participate in program and agency meetings.
Required Skills/Abilities:
Strong verbal and written communication skills with the ability to interact with individuals at all levels internally and externally.
Excellent interpersonal skills, including empathetic, kind, compassionate.
Able to effectively listen to clients to identify needs and/or roadblocks.
Analytical, organized, efficient, high attention to details.
Self-motivated and self-starter that can work with minimal guidance.
Ability to function well in a high-paced and, at times, stressful environment.
Driver's license and local travel required.
Education and
Experience:
Bachelor's degree (required).
3 years' experience in a similar role.
Prior experience working with individuals with disabilities and case management.
Knowledge of available community services a plus.
Proficient with Microsoft Office Suite.
